New Starter Pack

New members who have been offered places at COSH will need to complete the forms below before your child/ children can start to attend the setting.

Emergency contact and consent form

This form provides COSH with all the details needed about yourself and children in case of an emergency and what activities you agree for your children to participate in.

Parent contract

This is for parents and their children to agree to the terms and conditions of COSH.

Allergy, Dietary, Medical and S.E.N Form / Health Management Form

This form helps us to tailor our childcare to your individual child’s needs and requirements.

All about me booklet for children in nursery and reception

This helps COSH by providing further detail about your child, such as likes & dislikes.

Please note, you will also need to set up the standing order and make the first payment, the bank details are not available on the website but will be emailed to you upon accepted places.
